CONNECARE H2020 app for iPhone and iPad

Fundacio Eurecat as coordinator of the CONNECARE project (http://connecare.eu/) presents the CONNECARE H2020 app. It gives support to complex chronic patients in self-managing their diseases. CONNECARE monitors physical activities, health status, and allows communicate with the doctors in charge of the case.
